>An advance directive is a form you fill out to tell your family about your end of life wishes. It includes instructions on whether or not you want extraordinary measures to keep you alive, a designation of a person who will make health care decisions on your behalf if you cannot, and even directions about your funeral. Our office assists you with the Advance Directive free of charge when we are preparing other estate planning documents for you. If you want to fill out the form yourself, you can go to the Vermont Department of Health or the New Hampshire Foundation for Healthy Communities links. Be sure to give copies of the Advance Directive to the person who is designated as agent for your health care decisions, or let the agent know the location of the Advance Directive. Another alternative is to register your advance directive with the Vermont Department of Health link above, or with the U.S. Living Will Registry .
